YOU MUST REQUEST A "MINORS TRAVEL FORM" FROM US.
This form MUST be filled out and notorized BEFORE flying to Jamaica and taking part with GTMI.
If you are the parent of a minor and you are accompanying your minor child, your child is EXEMPTED FROM THIS.
